Please read this document carefully. This document describes the expectations for the Six Sigma Racial Equity Institute. If accepted into the program, you will be asked to sign this document to confirm your acceptance and agreement with these expectations.

Application Requirements

• I am a Black female or a Black Latina who works or lives in Indianapolis (Marion County), Indiana.

• I understand that completing an application is not a guarantee of acceptance.

• I understand that there will be more candidates compared to seats.

• If admitted, I understand there is a tuition fee. If not paid within 30 days from the invoice date, my position will be forfeited. 

 

Participation Requirements

• I understand that no refunds are available after admission. 

• I understand that even though I may have many responsibilities, my commitment to the course requirements is a priority. 

• I understand that the Institute expects a high level of professionalism, integrity and respect from all participants.

• I understand there are three parts for certification: class completion, pass (80%) a test, and project completion.  

• I understand that there are six 7-hour days of instruction. Instruction may be virtual or in-person. 

• I understand that I may only miss 1 day of class. 

• I understand that I am required to have a laptop to participate in the program. 

• I understand that I am expected to keep my camera on and minimize distractions for each virtual class.

• I understand that I am expected to read course materials and complete homework on time.

• I understand training classes, coaching meetings, and project meetings will occur during daytime hours.

• I understand that the time required for the project may be 100+ hours over five months.

 

Coaching

• I understand that at least 10 hours of coaching will be required during the project phase.

• I understand that it is my responsibility to schedule meetings with my coach(es).

• I will respect my coach's time by showing up for meetings and by providing sufficient lead time when scheduling meetings. 

• I commit to being coachable and open to constructive feedback. 

• I will ask questions immediately if I am unclear regarding course material or assignments.

 

Presentation

• I understand that I will be required to co-lead a team during the project phase. 

• I understand a presentation will be required upon completion of the project.

 

Marketing

• I agree to submit a photo for marketing purposes.

• I understand that my image may be used to advertise the Institute.
   

Post-Certification

• I am willing to complete a survey pre and post-certification. 

• I understand that this program may be part of a study that requires periodic surveys for assessing long-term impact.

 

If, at any time, I choose not to follow the expectations described in this document, I understand that I may be dismissed from the program without a refund.
